  1. One name per application (photocopy as necessary).
  2. Cheques or money orders must be payable to Grand Chapter of B.C. and Yukon O.E.S.
  3. If your cheque includes other members, please print their names on THE BACK OF THE CHEQUE
  4. ENCLOSE YOUR OFFICIAL CREDENTIAL FORM WITH CHAPTER SEAL. (In order to avoid any delays for pre-registered members, we are asking you to send in your credential with your registration form).
  5. At Grand Chapter you will be required to present your current dues card, which has been properly signed and showing the seal of your chapter, before you receive your Grand Chapter Card.
